Guidelines and Fee Schedule for Medicine●On●Time®
Here at Snell’s Pharmacy,
we offer the Medicine●On●Time®
packaging system, ideal for use in personal care facilities,
as well as for any patient who desires a more regimental and systematic
method to have multiple medications packaged.
Our packaging schedule is
based on a 28-day cycle*.
The due dates for upcoming cycles will be about the same time every month.
We guarantee** medications
delivered*** on the night
before the cycle begins. The process of preparing the bubble packed
medications is time consumptive and labor intensive. If delivery to
facility is required, we must have a valid credit card (Visa, MC, or
Discover) on file. We ask that a credit card be brought in to our facility
for an initial scan to log it into the patient’s profile. Charges will be
applied to credit card the night the medication leaves our facility and
receipts for all charged items will be mailed, in the name of the patient,
to the address we have on file. If you will be picking up and delivering
the medications in person, payment may be made with cash, check or credit
card. If you have Power of Attorney over a family member or loved one we
require a copy in the file in order to comply with HIPAA regulations so we
may discuss the patient’s therapy with you. To help us stay on course and
to fulfill our goals we have developed a guideline and fee schedule.
Medications can be changed
only by the orders of a Physician. If you wish to implement any changes,
please have the Doctor call with official medication orders. Orders changed
after a cycle has been delivered will be subject to a repack fee.
By Law, Medication that
has left the confines of the pharmacy cannot be taken back and re-sold.
This applies to Medicine-On-Time as well. There can be no refunds for
medication that is not resalable.
In developing guidelines
for this system, we have established 2 tiers of participation. The fees and
rules differ slightly between them. The guidelines and associated fee
schedules are as follows:
I.
Patients who fill medications through our facility:
We will package and deliver your medications for the cost of
the prescription plus a packing fee of $5.00 per Rx per month. This fee
will be on a separate MOT receipt along with the receipts for medications
filled. OTC medication will be done at a cost of $5.00 each, this includes
packaging and medication for most****
products.
We will be in contact with your physician to ensure the
medications have ample refills so that no stoppage in dosing occurs.
Medication and dosage changes will be taken care of as quickly
as possible. We will make same day adjustments on regimens as long as
notification is received by 5 O’clock, otherwise it will be done on the next
day. Friday and holiday changes will be done on the next business day, of
course this excludes life threatening situations, which will be handled
ASAP.
Changes in therapy that occur mid-cycle that require retrieval
and re-delivery of cycle pack for breakdown and repacking will be charged a
$5.00 per card repacking fee.
II.
Patients who do not fill medications through our facility:
Package and delivery fee for medications brought in from
outside sources will be charged $10.00 per prescription per month (For our
Honored Veterans the charge will be $5.00). This fee will be on a separate
MOT receipt.
OTC medication will be done at a cost of $5.00 each, this
includes packaging and medication for most****
products.
You will be responsible for knowing when the patient’s next
cycle is due. You must have all medications turned in to us no later than 7
days before the due date. We cannot and will not be held responsible for
medication delays and stoppages in therapy for medication that come from
external sources.
You will be responsible for maintaining refill records and
having ample refills from the MD to keep the regimen going. If we have to
call you or the doctor, there will be a processing fee of $5.00.
Any outside medication that comes back to our facility due to
status change will be stored for a maximum of 2 weeks then will be destroyed
according to the laws governing such drugs. Please notify us if you need to
arrange a pick-up.
We will require on file a signed waiver to allow us to use our
stock in the case of medication changes or medication shortage from the
outsourced prescriptions.
Changes in therapy that occur mid-cycle that require retrieval

We currently service over 150 patients in various
facilities, so it is the responsibility of the family or guardian to
notify us of any change in the patient’s status that would affect or render
un-necessary the preparation and delivery of medication. In the event
that the medication is packaged and delivered when unnecessary there will be
no refund for medication and packaging fees. In the event that medicines
are packaged, but not delivered, there will be a restocking fee, but the
price of the medication will be refunded.

not residential ****OTC medication that are higher in price may cost more
than the $10.00 charge
